SuperSport United sealed their spot in the Nedbank Last 16 after overcoming Cape Town City on penalties at the Lucas Moripe Stadium on Tuesday night.
City named new arrival Khanya Leshabela on the bench with youngster Heaven Sereetsi making his debut, while SuperSport welcomed back Ricardo Goss in goal after doing duty with the national team.
Matsatsantsa caused the first scare earlier on and draw a stupendous point-blank parry from Darren Keet following Etiosa Ighodaro's rasping attempt from a ball in-behind.
Rising star Shandre Campbell came good though when he applied a sharp-turn to bring a low cross from the left in his stride and finished in the bottom-corner.
SuperSport weren't given the time to relax following the lead, but had to keep a very tight line between defence and attack to stave off the visitors who looked to level.
Ighodaro was inches away from doubling the tally a little to the closing quarter of the half through a forceful header from Lyle Lakay's cross, but Keet parried.
However, the Citizens didn't stop pushing the Tshwane giants and were rewarded on the stroke of halftime when Khanyisa Mayo applied a smart finish from the area.
Eric Tinkler, who manages City, looked to the bench before the second half started and introduced Thabiso Kutumela ahead of Taahir Goedeman.
Keet kept the visitors alive yet again when he pulled around his area to parry Campbell's rasping strike from point-blank range.
Debutant Sereetsi continued to be alive on the ball past well ahead of the midway stages and was inches away from a goal, but he ended up going wide with a long-range pull.
With time flowing, City decided to give new man Leshabelah his debut and also introduced Angolan Jo Paciencia as no further goals forced the match to extra-time.
SuperSport squandered a brilliant chance to go ahead eight minutes in with a flying header by Bradley Grobler, but he went wide.
There were no winner in 120 minutes of action, and penalties were needed to determine who goes through to the last 16.
After going up first and converting through Lorenzo Gordinho, the Citizens missed the next three through Leshaela, Paciencia Thabo Nodada and left Siyanda Xulu to fire in the winning spot-kick for United.
